Choose from text inputs, dropdowns, checkboxes, file uploads, date pickers, and more. Add conditional logic to show or hide fields based on previous answers. Every submission is stored securely and can be exported as CSV for external processing.
Spam protection is built in with honeypot fields and rate limiting — no CAPTCHAs needed. Forward submissions to external systems via webhook, or connect directly with your email marketing tools to add respondents to subscriber lists.
